• Immunization Requirements for school entry:

    Students 11-years-old and in or entering the 6th grade must have a Meningococcal immunization and a DTap booster to attend school.

    DtaP/DT/TD – Diptheria, Tetanus, Pertussis
    *Must have 4 or 5 doses with the last one given after 4th birthday.
    *1st dose must have been given after 6 weeks of age or it is considered a non-dose.
    *8th-grade students must have the tetanus/diphtheria/pertussis combination (TDap) within the last 10 years. If it has been over 10 years, a booster will be needed to enter high school.

    IPV, OPV – Polio
    *Must have 3 doses at least.
    *Must have 4 doses if 3rd dose given before 4th birthday.

    MMR – Measles, Mumps, Rubella
    *Must have 2 doses.
    *1st dose after first birthday.
    *2nd dose before school entry (Kindergarten).

    Hep B – Hepatitis B
    *Must have 3 shots over 5 to 6 month period.
    *2nd dose 4 to 8 weeks after 1st dose.
    *3rd dose 2-5 months after 2nd dose.

    VZV – Varicella (Chickenpox)
    *All students should be immunized for Varicella. 6th-graders are exempt; however, they will need the Varicella booster to enter 7th grade. For convenience, it is wise to get this immunization with the required 11-year boosters.
    Hepatitis A may have been given but is not required to attend school.

    All immunizations must be current for school entry. (Above criteria must be met with no doses due.)

     

    Medications At School:
    Parents can bring in prescription and over-the-counter medications for their children provided the medications are clearly labeled in their original package and the proper forms are in place in the Nurse's office.
