Immunizations are required for students to register and attend school in Arizona (A.R.S 15-871 & 15-874). Below is a link to the Arizona School Immunization Requirements, Kindergarten through 12th grade. Please review this form and make sure your child has the appropriate immunizations for his/her age and grade. Also included are the requirements for students who are 11 years old.
I will review all the records and if your child is missing any immunizations required for school, I will contact you and let you know what is needed. For students that are turning 11 years old, I will reach out by mail before or near your child's birthday to remind you that additional immunizations are required for school attendance. You will be allowed a 10 day grace period after receiving a notice of missing immunizations to get the necessary shots. After the grace period, if you have not gotten the shots, then your child will not be allowed to attend school until they are brought up to date and the shots are received. After completing the shots, you must provide the nurse with a copy of the proof of immunizations. You can scan and email them, mail them or drop them off in the office.

FREE IMMUNIZATIONS ARE AVAILABLE! (for school required immunizations). When you go, bring your immunization record and insurance card so they may be reimbursed for your child's immunizations.
