ImmunizationsDisease Prevention/Immunizations: Written proof of compliance with immunization requirements is a part of the registration process in Arizona Schools. State and Federal Law mandates that school-age children be immunized to prevent certain diseases. If you are unsure of the requirements for Immunization, see your doctor, visit the School Health Center or call the Maricopa County Health Dept at (602)-506-6767 or (602)-263-8856 for locations of clinics and hours of operation. Immunizations are free of charge for children under the age of 18. Exemption forms are available from the nurse.For Childcare, Preschool and Kindergarten in 2014-2015:For Kindergarten to 12th grade in 2014-2015:http://azdhs.gov/phs/immunization/documents/school-childcare/2014-2015-school-year-immunization-requirements.pdfA message to parent(s) or guardian(s) of incoming 6th graders:Arizona Department of Health Services requires that students going into the 6th grade will need two more immunizations for school attendance. Students 11 years of age and older and entering 6th grade are now required to have Tdap and MV to attend school. These immunizations are recommended for students in 5th grade as they turn 11 years old as well as older students entering 7th grade and above. Letters were sent out to all returning students about these requirements before summer. If you haven't already, please make sure to bring proof of immunizations to me as soon as possible. For your convenience, free shots are offered by the Maricopa County Immunization Clinics at several valley locations.
