• Immunization Requirements

    According to Arizona Revised Statutes (ARS) §15-872 (B), states that "a pupil shall not be allowed to attend school without submitting documentary proof to the school administrator unless the pupil is exempted from immunization pursuant to section 15-873".Students of all ages are required to have immunizations to be enrolled in school.  Pre-school, kindergarten and 6th grade have very specific immunization requirements.  Please visit the azdhs.gov website and look under immunizations for additional information that outlines age and grade-specific state-required immunizations. Exemption forms are available in the nurses office. There is a religious exemption for pre-school, a personal exemption for grade school and a medical exemption that must be filled out by a physician stating that a student had a medical condition that prevents the child from receiving vaccines.If an illness breaks out and your child is not immunized, they may be asked to remain out of school for up to a three-week period. The Maricopa County Health Department has a free immunization clinic at 1850 N. 95th ave., Suite 184 Phoenix, Az. 85037.  Their phone number is 602-506-6767.  They are open Monday through Friday from 8 am - 12:30p and 1:30 pm to 5pm (closed on weekends and holidays) .  We also have Mountain Park Health Center in Sunrise Elementary School at 17624 N. 31st avenue and their number is 602-323-8048 (they accept most insurances and have a sliding scale option for no insurance).
