• COVID-19 Reporting Procedures for

    DVUSD Parents and Students


    If a student has symptoms of COVID-19, tests positive for COVID-19 or if a student has come into close contact (within 6 feet for 15 or more minutes within a 24 hour period) with someone who is positive or potentially positive with COVID-19, parents should do the following:

         *Keep child home.

         *If your child tests positive or has COVID-19 like symptoms and has had close contact with any sibilings who attend any DVUSD schools, then your child and all other siblings should be kept home as well.

         *Parent is to complete the Student COVID 19 Information Form at:  https://url.dvusd.org/COVID19ReportingForm.

         *Contact the School Nurse and a designated campus administrator via email, informing them of the situation regarding your child's health. PLEASE specify in the email if the situation involves a positive test, suspected positive test (based on child's symptoms) or if they have had close contact with someone who has had a positive test or suspected positive test. Please also include in the email: Name of student, student school ID number, date of COVID test and date that the symptoms began.

         *Complete the regular student absence procedures by contacting the school attendance line at 623-445-3390 and notifying them of student's absence.

         *If a parent has any questions, they can email COVID19concerns@dvusd.org or they can contact the School Nurse via phone (623-445-3310) or email grace.agapay@dvusd.org.

         *A district representative will follow up with the parent regarding the received report and will continue contract tracing data collection.olationtool
