Summertime!!!! In addition to having a relaxing and fun-filled summer, please be aware of the following as you are scheduling doctor appointments/check-ups:
- If your student turns 11 between 5/20/23 and 7/30/23 they are due for immunizations per Arizona Department of Health Services. https://www.azdhs.gov/documents/preparedness/epidemiology-disease-control/immunization/school-childcare/school-immunization-requirements.pdf
Proof of these immunizations will be required upon the student's return to school.
- Complete health information packets for those students with Diabetes, Seizures, Asthma, and Severe Allergies. These forms need to be filled out at the beginning of every school year and given to the nurse. Copies of these packets are in the "Forms" section of the Health Office link on the Diamond Canyon webpage.
- ALL Prescription medications to be given at school require a DVUSD form signed by the ordering physician, without exception. You can print the "Medical Authorization to Administer Medication" form from the "Forms" section of the Health Office tab on the Diamond Canyon webpage.
