    Required Documents
    Children entering Kindergarten for the first time must be five (5) years old by August 31st. Children entering the first grade must be six (6) years old by August 31st. When registering any student at Sunrise, the following documents are required:

    • Certified copy of the student's birth certificate.
    • Immunization record for the student.
    • Proof of residency.
    • Legal papers in regard to custody, name change, adoption (if applicable)

    Immunization
    Arizona law requires that all children entering school be immunized. Verifiable documentation is required of all new entrants to Arizona schools. This documentation must show the date and type of dose administered or laboratory evidence of immunity.

    Emergency Cards
    All parents must complete an Emergency Medical Referral Card for each of their children every year. This card will tell us how to contact you or another responsible adult in the event your child becomes ill or is injured at school. Students will be released early due to illness or personal reasons ONLY to those persons listed on the emergency card. A note or telephone call is NOT sufficient to release your child to persons other than those listed on the card. Identification will be required of ANYONE picking up a student early from school. Please inform the school office immediately if any changes in contact information need to be made.

    Custody
    If there is a court order that limits the rights of one parent in matters of custody or visitation, a copy of that order must be on file in the school office. Unless that court order is on file, we must provide equal rights to both parents. In the event that you leave your child in the temporary custody of a friend or relative, the school must have a notarized letter. Hospitals require this notarized information in the event of an emergency. In addition, please be sure the friend or relative is also listed on the student's Emergency Medical Referral Card at school.

    Open Enrollment
    Open Enrollment, a program enacted by the Arizona Legislature, requires all school districts to allow students the opportunity to select a school of their choice if capacity permits. DVUSD complies fully with all provisions of the statute.
