• Dear Parents/Guardians,

    Although students are not currently on our campus. The mandates set forth by the Arizona Department of Education and Arizona Dept. of Health Services require that the Health Office Nurse continue to conduct hearing and vision screenings for students that are new to the district within 45 days from the first day of school.  Students receiving DVUSD Special Education Services and All Kindergartners will also be screened during this time.

    I will start scheduling Screening appointments for September 2020.

    Times will be available between  8:30 am to 3:00 pm. I can accommodate some appointment if needed, just let me know.

    1. Appointments will be scheduled 30 minutes apart.  Parent/Guardian will arrive with child, both wearing masks.  Prior to screening, you will complete a short COVID-19 questionnaire before screening.

    2. The screening will be conducted in the Health Center.  Only one student per time slot.  If you have a sibling that will also need to be screened, we can schedule them around the same time so you do not have to come another day. Physical distancing will be maintained while waiting.

    3. Student will not be allowed in the main building at any time.

    I look forward to meeting you and your child. This is a chance for them to come on campus even if it's just briefly and for me to meet them as well.

    I will take the upmost care to insure safety for you and your child per CDC and ADHS protocols. 

    Hearing and vision screening is an important factor in your child's education and development.
