All 6th Grade Students who are at least 11 years old are required by Arizona Law to have the following additional immunizations before returning to school. 

1. Meningococcal

2. Tdap dose if it has been more than 5 years since the last DTdap, DTP, DT, or Td dose. 

If your child will be a 6th grader and is at least 11 years old, please bring documentation from your Doctor's office, clinic, or hospital to the Nurse's Office. Documentation should include the date, name of the doctor, clinic, or hospital, and the immunization(s) given.
