Parent/Guardian COVID-19 Acknowledgement & Disclosure Form
Prior to your child returning to on-campus learning, you will need to complete the Desert Sage Elementary COVID-19 Parent/Guardian Acknowledgement and Disclosure Form. A form will need to be completed for each individual child attending Desert Sage. Please print the form, fill it out and sign, then either drop the form off at the Desert Sage office anytime Monday through Friday, 7:30 a.m. - 4:00 p.m. or scan and email the form to firstname.lastname@example.org. If you are unable to print the form, there will be copies available at the Desert Sage office.

If this form is not received by the fifth day of on-campus learning, your child will NOT be permitted to return to on-campus learning until it is received. The form due date for each grade level is outlined below.
